H.R. 2437 — EASE Act of 2025

This section revises Medicare hospital discharge planning requirements under which hospitals must provide certain inpatients with information on available post-hospital services by (1) specifying, for individuals likely to need home health services, the availability of such services through home health agencies; (2) requiring, for individuals likely to need post-hospital extended care services (e.g., skilled nursing facility care), information on providers listed by the hospital as available; and (3) requiring, for individuals likely eligible for hospice care, information on Medicare-participating hospice programs serving the patient's area. The amendments apply to discharges occurring on or after January 1, 2026.
