H.R. 6735 — Connecting Caregivers to Medicare Act of 2025

This section requires the Secretary of Health and Human Services to conduct outreach and education activities to inform Medicare beneficiaries entitled to Part A benefits or enrolled in Part B (and applicants) about the option to authorize a family caregiver to access their personal health information through the 1-800-MEDICARE toll-free number using CMS form 10106 (or successor). The activities must include (1) outreach to beneficiaries, providers and suppliers, family caregivers, and other entities specified by the Secretary; (2) dissemination of best practices, to be developed by the HHS Office of the Inspector General within one year of enactment, to protect beneficiaries from fraud involving unauthorized access to their information; (3) promotion of Medicare fraud reporting mechanisms; (4) prominent inclusion of related information and the authorization form in the Medicare & You handbook, on Medicare.gov, in Medicare Advantage plan efforts, and on other platforms such as social media; (5) training for 1-800-MEDICARE operators to assist family caregivers; and (6) feedback opportunities such as surveys for family caregivers. The section further requires multilingual materials (as specified by the Secretary), coordination with State Health Insurance Assistance Programs and the Administration for Community Living to the extent practicable, clarification that the option applies regardless of whether beneficiaries are in original Medicare or a Medicare Advantage plan, and definitions of family caregiver (per the 2017 Recognize, Assist, Include, Support, and Engage Family Caregivers Act, plus others specified by the Secretary) and specified family caregiver information access authorization form.
