H.R. 786 — Preserving Patient Access to Accountable Care Act

This section extends additional incentive payments under Medicare Part B (i.e., 42 U.S.C. 1395l(z)) to eligible professionals participating in eligible alternative payment models (APMs)—payment arrangements that bear a meaningful level of downside financial risk—through 2027 (from 2026), specifying a payment amount equal to 3.53% of the professional's estimated aggregate Part B payments for 2026 (in addition to the existing 1.88% rate). The section also (1) updates the definitions of qualifying APM participation years and payment years to 2027 and 2028, respectively, and (2) makes conforming amendments to the exclusion from the merit-based incentive payment system (MIPS) for such APM participants through 2028 (from 2027).
