S. 247 — Choices for Increased Mobility Act of 2025

This section establishes special rules for ultralightweight manual wheelchairs under Medicare Part B durable medical equipment (DME) payment provisions (42 U.S.C. 1395m(a)), effective for items furnished on or after January 1, 2026. Specifically, the section (1) requires the Secretary to establish two or more Healthcare Common Procedure Coding System (HCPCS) codes for the wheelchair base depending on construction material (one or more codes for titanium or carbon fiber and one or more for other materials); and (2) for wheelchairs with titanium or carbon fiber bases, provides that Medicare pays suppliers the standard amount (80% of the lesser of the actual charge or fee-schedule amount), permits suppliers to charge beneficiaries the difference between that payment and the supplier's actual charge, and authorizes the Secretary to require suppliers to provide advance notice to beneficiaries of potential financial liability. (Thus, these wheelchairs are excepted from standard DME supplier assignment requirements, potentially increasing beneficiary out-of-pocket costs for advanced-material models.)
