S. 2612 — SAFE Act

This section expands the Medicare annual wellness visit to include, for beneficiaries determined by a physician to have fallen in the previous calendar year, a falls risk assessment and fall prevention services; and authorizes a physical therapist or occupational therapist to furnish such services separately. This section further includes outpatient physical therapy services and outpatient occupational therapy services in the initial preventive physical examination for such beneficiaries. The changes apply to annual wellness visits and initial preventive physical examinations furnished on or after January 1, 2026.

This section directs the Secretary of Health and Human Services to submit to Congress, beginning January 1, 2027, and annually thereafter, a report on the number of falls (as reported by the Centers for Disease Control and Prevention) experienced by individuals aged 65 and older who received treatment for pain or injury related to a fall or falls in the previous calendar year. Each subsequent report must include data from previous years and describe any year-to-year changes.
