This section establishes Medicare Part B coverage, effective for services furnished on or after January 1, 2027, for a preventive home visit (as defined in new SSA §1861(nnn)), conducted not more frequently than once every 2 years by a qualified professional or team to assess the beneficiary's home environment, identify health risks, and provide referrals for interventions or home modifications to improve physical function, balance, strength, mobility, reduce falls risk factors, or enhance nutrition (in-person, remotely, or a combination). It provides payment at 100 percent of the lesser of the actual charge or a bundled amount established by the Secretary under new SSA §1834(aa) (which includes any related referrals), with no coverage if furnished more frequently than authorized.
