S. 3439 — Connecting Caregivers to Medicare Act of 2025

This section directs the Secretary of Health and Human Services to conduct outreach and education activities to inform Medicare beneficiaries entitled to Part A benefits or enrolled in Part B (and applicants), as well as providers, suppliers, family caregivers, and other entities, about the option to authorize a family caregiver—defined under the Recognize, Assist, Include, Support, and Engage Family Caregivers Act of 2017 (or as specified by the Secretary)—to access the beneficiary's personal health information via the 1-800-MEDICARE toll-free number using CMS form 10106 (or successor form). Such activities must (1) include best practices, to be developed by the HHS Office of the Inspector General and submitted to Congress within one year of enactment, to protect beneficiaries from fraud involving unauthorized access to their information and to promote fraud reporting; (2) provide information on existing Medicare fraud reporting mechanisms; (3) feature the option and form prominently in Medicare notices, on Medicare.gov, in Medicare Advantage communications, and on other platforms (e.g., social media), in multiple languages; (4) clarify availability regardless of fee-for-service or Medicare Advantage enrollment; (5) train 1-800-MEDICARE operators; (6) solicit feedback from family caregivers; and (7) coordinate with State Health Insurance Assistance Programs and the Administration for Community Living.
