S. 4481 — IMPACT to Save Moms Act

This section establishes the Perinatal Care Alternative Payment Model Demonstration Project for fiscal years 2027 through 2031 to allow states to test alternative payment models for maternity care provided to pregnant and postpartum individuals enrolled in Medicaid and CHIP. The Secretary of Health and Human Services must coordinate with state Medicaid programs, maternity care providers, patient and community organizations focused on groups with elevated rates of maternal mortality and disparities, perinatal health workers, insurers, hospitals, birth centers, federally qualified health centers, rural health clinics, and researchers when designing the project. The Secretary must consider models that stratify patients by pregnancy risk level with adjusted payments, use evidence-based quality metrics, incorporate non-hospital settings and social determinants of health, include diverse maternity care teams with implicit bias training, and address maternal mental health and substance use disorders. States participate by submitting applications, and the Secretary must evaluate impacts on maternal health outcomes (stratified by race, ethnicity, language, socioeconomic status, geography, and insurance type), state spending, and patient experience, then report results and recommendations on national expansion to Congress within one year after the project ends.
